This is a pattern finder, not a diagnosis. Check every sign that applies to the student. The score helps you decide whether to monitor, act early, or request a structured academic review.

No email is required. Your score is calculated in your browser, so families, schools, aftercare teams, and support providers can use the checklist as a neutral observation tool.

Inspired by MTSS/RtI pattern-based screening logic: one concern is not a diagnosis; repeated signs across learning areas may be a reason to consider a deeper academic review.

No. This is an educational pattern-finding tool, not a diagnosis. It helps organize observable signs so families, schools, and support providers can decide whether a deeper academic review may be useful.

Who can use this checklist?+

Parents, teachers, aftercare teams, preschools, daycare directors, therapists, and academic support providers can use it as a neutral observation tool. No signup is required.

My child's grades are fine. Should I still use this?+

Yes. Grades can sometimes hide early gaps because students may receive scaffolding, retakes, partial credit, or extra help. This checklist looks for repeated patterns, not report cards alone.

How should I interpret the score?+

Use the score as a starting point for discussion, not a final answer. A higher score means more repeated signs were selected in grade-relevant areas. The exported report gives a clearer section-by-section interpretation.

What should I do with the patterns identified?+

Review the exported report, compare it with homework or classwork, and share the observations with a teacher, school program lead, aftercare director, therapist, or academic support provider.

Can I share this checklist with families or staff?+

What grades does this checklist cover?+

The checklist is grade-aware for elementary and middle school readiness signs. GrowWise serves grades 1-12, but the checklist score uses only the sections relevant to the selected grade band.

What if something in the report looks incorrect or unclear?+

The report is generated from the signs selected in the checklist. If anything looks incorrect or unclear, email contact@growwiseschool.org and we'll review it.
